Instructions, Terms and conditions

• Please fill in your contact details ensuring you don't miss any form field out below under these instructions.
• Packaging. ensure you pack your Artwork in a strong cardboard box so your artwork won't be damaged.
• Artist Code, on the front the top right corner in large letters write your Artist code in felt marker pen Write in Dark Blue or Black preferably using a vivid marker so the ink won't run if the box gets wet.
• Artist code is the first 3 letters of your Surname and last 3 numbers of your Mobile phone number
• Write your name, Address and Artwork title in large letters on the front of your packaging box. This all makes packing and unpacking easier for the set up team.
• Commission, probably 30% commission payable to DOC upon the sale of your Artworks - to be confirmed.
• Entry Fee $20 per Artist for all entries - fees go to marketing and promotion.
• Please make payment before submitting your form.

Your entry fee money is held in escrow until the exhibition starts.
•Use your artist code as a reference. Generate your Artist code on the form below
• Include your bank account name and and account numbers for payment of any sales on the entry form below. Please ensure your bank account name is accurate.
• Paper copy for each artwork. download a template of the fom here at this link

• The paper copy of your entry form is for sticking on the back of your Art work and on the wall next to your Artworks at the exhibition. You can fill this in by hand and print them off or, Copy the text from your entry details for each Artwork into the form below. Paste in your PDF annotator. If you don't have a pdf annotar.

Open the pdf entry form that when printed will be your paper version of the entry form and in your Pdf Annotator, select comment in box and place a text box in each place you want to paste text and your entry form text from this online form which you copy (highlight Control C for PC or Command C for Apple) if you get stuck doing this give me a call on 021 109 8290 and I'll help you do it. on the phone.

• Be sure to mention if your Artwork has won an award as it has benefits for getting a better page ranking online when we later copy this text over to the website.
